Formula used

Each KOOS subscale is scored separately.

Raw Sum = sum of all responses inside one subscale.

Mean Response = Raw Sum / Number of Items.

Transformed Score = 100 - (Mean Response × 25).

An equivalent form is: Score = (1 - Raw Sum / (4 × Number of Items)) × 100.

Higher values indicate fewer symptoms and better function. Standard KOOS interpretation focuses on the five separate subscales rather than one official total score.

Frequently asked questions

What does KOOS measure?

KOOS measures symptoms, pain, daily function, sport or recreation function, and knee related quality of life. It helps summarize how a knee condition affects routine activity and demanding movement.

Does a higher score mean a worse knee?

No. Higher KOOS scores mean fewer symptoms and better function. Scores closer to 100 reflect better knee status, while lower scores suggest more pain, limitation, or quality of life impact.

Is there one official KOOS total score?

Official KOOS reporting uses five separate subscale scores. This page also shows an optional average summary for quick review, but clinical discussion should focus on the individual subscales.

Do I need every item completed?

Yes. This version expects all items to be answered so each subscale is scored consistently. Complete every response before calculating to avoid distorted domain results.

Can this replace a clinician evaluation?

No. KOOS is a structured patient reported outcome tool, not a diagnosis. Use it to support progress tracking and discussion alongside professional assessment, examination, and imaging when needed.

How often can I repeat the calculator?

You can repeat it whenever symptoms change or at planned follow up points. Consistent timing, such as weekly or after rehabilitation milestones, makes comparison easier.

Are CSV and PDF files stored online?

No. CSV and PDF files download to your device when selected. They are not automatically sent anywhere by this page.

Can I use KOOS after surgery or sports injury?

Yes. KOOS is commonly used for ligament injuries, meniscal issues, osteoarthritis, and recovery tracking. Interpretation should still consider diagnosis, healing stage, rehabilitation goals, and clinician guidance.
